Information about adverse reactions, side effects and contraindications will be supplied with all medication and can usually be found on the packaging of over-the-counter drug products and on package inserts or printed materials dispensed with prescription drugs with detailed information on action to take and when to seek medical advice.

Adverse reactions

This is a negative reaction to a medication and can be either mild (rash) or very serious (anaphylactic shock), but will not occur in every person taking that medication or on every occasion.

You must always check for known adverse reactions in the person’s care plan. If you witness an adverse reaction you must record this in the care plan.
