You decided that Gillian is the best person to complete the incident report and are confident that she will do this. Two days later, you are in a hand over meeting when Jane discusses Bill.

Option 1: At the time, you say nothing as Gillian is not present. You feel it would be inappropriate to say anything without speaking to Gillian first. The next day, you speak to Jane to let her know what had happened.
Option 2: You tell Jane about the incident, that you had spoken to Gillian, and she had assured you the paperwork had been done. You note that you have not written anything down.
Option 3: As you hadn’t seen Bill struggling to eat, you contact Gillian to let her know what has happened and tell her that they cannot find the incident report.
